Circular incision

The incision is made around the perimeter of the areola resulting in a highly discreet scar. A circular incision works best for less extensive revisions, as access to the breast tissue and skin remains limited.

Inverted-T

Inverted-T incision

The inverted-T, also known as the anchor incision, provides comprehensive access to the breast area and is suitable for moderate to substantial reduction procedures. This incision extends down vertically under the nipple and across the crease at the base of the breast mound.

Lollipop

Lollipop incision

A lollipop incision goes around the edges of the areola and extends vertically down to the crease of the breast. Good candidates have a moderate amount of excess breast tissue and skin.

How to Prepare for Best Results

Dr. Kopchinsky regularly performs surgery at CHRISTUS Santa Rosa Hospital in San Antonio, which has earned accreditation from the Joint Commission based upon the hospital's elite levels of safety and care standards. Our doctor will give you personalized instructions to follow before your arrival at the hospital. Common pre-operative guidelines include:

  • Receiving a baseline mammogram that will be compared to future mammograms to monitor breast tissue health. 
  • Stopping smoking a month before your surgery. Doing so lessens the risk of a variety of surgical complications. 
  • Modifying medications in consultation with Dr. Kopchinski. For example, blood thinners and anti-inflammatories should be avoided.

A Closer Look at the Procedure

Depending on your treatment plan, we may schedule your plastic surgery at our San Antonio outpatient facility or a local hospital to keep you under observation for the first night. Breast reduction typically takes between two and five hours and consists of the following steps:

Anesthesia

We begin by administering general anesthesia, inducing a sleep-like state before your surgery.

Incisions

Dr. Kopchinski makes incisions around the areola that extend down the breast mound in cases involving more extensive revisions.

Cosmetic Modifications

Next, we remove excess tissue and trim away loose skin. We may also incorporate liposuction to eliminate a greater amount of fat in the region. The skin is redraped over the chest and pulled tight, and the nipple repositioned higher up on the breast mound.

Finishing Touches

When the reduction is complete, we close the incisions with sutures and place a surgical drain to prevent fluid buildup in the first few days. Finally, the chest is bound in gauze to protect the incisions as you recover.

What to Expect During Recovery

We ask that you carefully follow all postoperative recovery instructions to prevent complications and ensure the best aesthetic results. Recovery typically involves:

Medications

You can anticipate certain side effects like swelling, bruising, and soreness around your chest in the days following surgery. Dr. Kopchinski may prescribe pain medication or other antibiotics to reduce discomfort and the chance of infection.

Breast Care

We will provide you with a surgical bra (a chest compression garment) to wear in the first week to improve circulation and support the chest as you heal. Change your bandages as instructed and avoid wearing any clothing that could irritate the area.

Downtime

Get plenty of rest in the first few days after your surgery. You will be at a reduced capacity initially, so you might want to plan for a friend or family member to assist you around the house, particularly in the first 48 hours. You can return to work with the doctor's permission in about one to two weeks, but avoid exercise and strenuous physical activity in the first month.
